Policies are necessary to ensure fair business practices are being followed for all clients,
and gives all clients an equal chance to receive services.

Please take a moment to review them carefully.

Scheduling:Your appointment is scheduled as Time Reserved Exclusively for You.
I do not take walk-ins that would take up your scheduled time.
Appointments will begin and end as scheduled.
You may schedule future
visits 1 month in advance.

Payment: Full payment is due at time of service.
Cash or Personal Checks are accepted.
Credit card payments will
add a $3.00 processing fee (subject to change)
Insufficient fund fees associated with a personal check, will need to be paid in cash.

Rates: I charge a fair rate for my services; Rate adjustments are reviewed and may change yearly.

Time: I allow 10 minutes after completing your session for you to redress, make payment, and
reschedule before the next client will arrive.

Tardiness: If you are late, full payment for your reserved appointment is still due.
Please arrive timely to avoid shortening your session.

Cancellations or Changes to your Appointment:
A 24 hr Minimum Cancellation Notice is Required By Phone Call.
This is a reasonable time frame in which I may have to fill the opening.

How to Cancel:  Within 24 hours of your scheduled appointment time,
call 919-389-5103. If leaving a voice mail, be specific with your verbiage.
Please state your name, followed by,"I am cancelling my appointment
scheduled for (date and time)".
I will return your call to confirm I have received your notice.

No Shows or No Cancellation Notice:
Full payment of your scheduled appointment will be due.
Future appointments that were reserved for you will be cancelled.
Once payment has been received for your missed appointment,
rescheduling may be permitted with certain restrictions, or declined at my discretion.

Socializing: I cannot socialize while I am working as I need to prepare and regenerate for the next
clients' session. Thank you for respecting this.

Privacy Policy: I maintain confidentiality of all client information per Rules and Regulations of the
NCBMBT; Confidentiality Roles and Boundaries section .0505.
This is necessary to manage a trusting client/therapist relationship.
Please refrain from asking information about another client even though they may be a friend or
close relative that you have referred.

Gift Certificates: Gift Certificates are transferable.
They are Non-Refundable. S&H, surcharges, or any processing fees for online
purchases added in the price and are not refundable.

Itemized Statements: I will provide you with an itemized bill suitable for flexible spending account
or insurance reimbursement upon request.

Insurance: At this time I do not file for insurance reimbursement of any type, with exception of
Workers Compensation, with proper prescription and prior authorization.
Payment is due at time of service.

Guests:  For safety and security, please do not bring children with you to your appointment.

This is a non-smoking, non-alcoholic, and non sexual environment,
Smoking, sexual harassment, or intoxication, is not tolerated.


Definitions:
Client- a client is a person to whom I have provided massage therapy services.
Assessment/Consent- the health intake form, that is required to be completed prior to your session.
Contraindication- conditions in which massage therapy is not recommended.
Massage and Bodywork Therapy- Systems of activity applies to soft tissues of the human body for
therapeutic, educational, or relaxation purposes. The application may include:
a. Pressure, friction, stroking, rocking, kneading, percussion, or passive or active stretching within
the normal anatomical range of movement.
b. Complementary methods, including the external application of water, heat, cold, lubricants, and
other topical preparations.
c. The use of mechanical devices that mimic or enhance actions that may possibly be done by the
hands. Practice of Massage and Bodywork Therapy- The application of massage and bodywork
therapy to any person for a fee or other consideration. "Practice of massage and bodywork
therapy" does not include the diagnosis of illness or disease, medical procedures, chiropractic
adjustment procedures, electrical stimulation, ultrasound, prescription of medicines, or the use of
modalities for which a license to practice medicine, chiropractic, nursing, physical therapy,
occupational therapy, acupuncture, or podiatry is required  by law.
